Requests For Support of a Charitable Contribution


General Guidelines:

  • Request must be from a 501(c)3 organization (documentation required)
  • Applications must be submitted online. Hard copy or emailed requests will not be considered.
  • Applications will only be accepted during January, March, May, July, September, and November.

Application Requirements

In addition to completing the online application form, applicants must provide:

1. W-9 Form (must be signed and cannot have PO Box as only address)

2. Non-discrimination statement or clause
3. Annual operating budget
4. List of Board of Directors, Trustees and Key Staff
5. Project Budget
6. IRS 501(c)3 Determination Letter
7. Audited financial statements (for requests of $10,000 or more)
8. Signed cover letter (OPTIONAL)


To Submit Your Request:

In order to submit a request, you will need to create an account with the Tax ID of your organization, email address, and password. You will be required to upload electronic versions of the documents listed above as part of your application.
